If you choose to direct bill with your primary insurance, please bring your insurance information and a doctor’s note (if required) to the appointment. A receipt for the secondary coverage will be available if needed.

Direct billing is offered as a courtesy, not a requirement. If the insurance submission doesn’t go through or the full amount isn’t covered, you will need to pay out of pocket. ALWAYS BRING ANOTHER FORM OF PAYMENT.
